I understand infection, irritation, allergic reaction, and scarring are possible, especially if aftercare is not followed.

I release DOTI Gallery Tattoo, DOTI Gallery LLC, owners, artists, employees, contractors, and agents from claims connected to design, placement, spelling, color, size, healing, or final outcome.

I acknowledge DOTI Gallery Tattoo may refuse service at any time.

I acknowledge I have had the opportunity to ask questions about receiving a piercing at DOTI Gallery Tattoo.

I understand piercings may reject, migrate, scar, swell, become irritated, or become infected.

I understand proper aftercare is required for healing.
